I pray that your family will get counseling, not just your nephew, but family counseling as well. If the school is this involved, perhaps they will have your nephew come into the counselor's there once a week or so? If your sister won't take him to a counselor this could be an option.
One thing about it, in my experience someone who has been hurt by suicide so bad doesn't usually think of that as an option. They have lived the other side and don't want to put anyone else through that. It sounds like you are close to your nephew. Could you arrange for a "him and you" trip somewhere? Something just to remind him that you are always there. I think the biggest thing here is availability. I wish you the best.
