You should contact the person that is in charge of the insurance at his company, since this is an issue of your health (teeth are important ;D) then they have to give you the info, If they do not, then I would contact a lawyer.
Do not let him bully you, He wants to feel in control and you have to take the control from him.
Make the moves to let him know this is not a game.
